1.1 The client is requesting, at her own express wish, the performance of the services specified in detail under § 2. The client is aware that the services mentioned are not part of the statutory health insurance benefits catalog and that the stated amount must therefore be borne by her.
1.2 Should the insurer decline to cover the costs of treatment, the practice will not provide justifications for the necessity of the treatment or otherwise contact the insurer. Furthermore, invoices already issued will not be subsequently amended at the insurer's request.
1.3 hermaid (Her-Medical-Aid GmbH) provides the platform and technical infrastructure to enable an online consultation with a specialist.
2.1 The services comprise consulting services provided by menopause specialists (doctors and licensed alternative practitioners) on the topic of menopause, aimed at providing orientation and support.
3.1 Treatment services are invoiced in accordance with the official fee schedule for physicians (GOÄ) under § 5 et seq. para. 1 GOÄ. Some treatment services are provided partly outside the scope of the GOÄ. For licensed alternative practitioners, billing follows the fee schedule for alternative practitioners. Coverage by private health insurers cannot be guaranteed. Costs not covered by the private insurer must be borne by the client herself.
3.2 The remuneration examples shown on the website are only examples for the most common services. Other treatment needs may arise in individual cases. Deviating costs will be discussed with the client in advance.
3.3 As a self-payer, the client agrees to pay the fee according to the rates set out in § 2 of this agreement. Services that go beyond the scope of medically necessary care are expressly provided at the patient's request (cf. § 1 para. 2 GOÄ). These invoices will be marked accordingly (cf. § 12 para. 3 GOÄ). Depending on the effort involved, the services may exceed the 2.3-fold rate.
3.4 Remuneration becomes due under § 12 GOÄ upon booking the service package through the hermaid platform. Payment is made in advance.
3.5 The patient has no entitlement to services outside the agreements concluded. If further questions arise after an appointment, a new appointment must be scheduled under the usual terms.
3.6 Additional services outside the scope of the GOÄ must be borne by the patient herself.
4.1 During the booking process on the hermaid platform, the patient can choose between various payment methods.
4.2 Payment processing is handled through the hermaid platform and the payment provider Stripe.
5.1 The client generally has the option to reschedule the appointment up to 2 hours in advance, thereby booking a new appointment.
5.2 Cancellation of the appointment is possible up to 48 hours before the scheduled appointment. The amount paid will be refunded by the 15th of the following month.
5.3 Cancellation or rescheduling of the appointment is no longer possible starting 2 hours before the scheduled appointment. In this case, no refund of costs can be made.

6.1 The client acknowledges that data is collected via the hermaid platform and made available to the doctor or alternative practitioner. The doctor or alternative practitioner uses the hermaid platform for documentation purposes.
6.2 Appointment booking and appointment management take place exclusively through the hermaid platform.
6.3 The client has been informed that, in connection with this agreement, data about her person, her social status, as well as the medical data necessary for treatment, will be stored, processed, and used, and may be transmitted to third parties (e.g. cost bearers, co-treating or referring physicians, rehabilitation facilities, etc.) within the scope of the intended purpose and in compliance with applicable data protection regulations. This typically concerns the data defined under § 301 SGB V.
6.4 The client is informed that, as part of the ongoing development of the hermaid platform, it cannot be entirely ruled out that, in exceptional cases, her data may become known to employees in pseudonymized form, who are bound by confidentiality obligations. The client hereby gives her consent to this.
7.1 If the appointment is cancelled less than 2 hours in advance, or in the case of a no-show, the full expected fee becomes due.
7.2 External findings are only accepted in digital form (PDF document) via upload on the hermaid platform.
7.3 Appointments to discuss new lab results are scheduled by the patient. Direct contact regarding new lab results is only made in cases of severe and acute pathology.
7.4 Consultation appointments are scheduled with an appropriate amount of time to ensure the client has a calm, unhurried atmosphere for the consultation. If the case-specific questions do not fill the allotted time, additional questions may be discussed. If the client ends the appointment early, this has no effect on the amount billed.
By giving my consent, I confirm that I have read and understood the treatment agreement, and that I am entering into a binding treatment relationship with the respective doctor or alternative practitioner.
