Application for Cost Reimbursement
Here you'll find a sample letter you can use as a template for your reimbursement request to your health insurer.
The letter can be filled out, printed, and sent by post. Many health insurers can also be reached by email. You submit this application yourself – no need to go through your doctor's practice.
Doctor's Recommendation Letter
Optionally, your doctor can give you a recommendation for hermaid as complementary support alongside your treatment.
Here you'll find a text template you can give to your doctor. Please have the letter signed and stamped by the practice – only then does it count as an active medical recommendation to your health insurer.
